A proposed return to the cage for two of women’s mixed martial arts trailblazers has become as much about medical clearance as competitive rivalry. As Ronda Rousey prepares to face Gina Carano in a five-round featherweight contest, stricter concussion and neurological testing has emerged as the decisive factor in whether the fight proceeds.
The bout is scheduled for May 16, 2026, at the Intuit Dome in Los Angeles and is set to stream live on Netflix. However, approval from the California State Athletic Commission (CSAC) depends entirely on both athletes successfully passing enhanced medical examinations.
Why the Testing Is Stricter
Both fighters are returning after lengthy absences from professional competition. Rousey, 39, last competed in MMA in 2016, finishing her career with a 12-2 professional record, including nine submission victories and three knockouts. Carano, 43, has not fought since 2009 and holds a 7-1 record, with three wins by knockout and one by submission.
Given their ages and time away from active competition, CSAC has mandated additional safeguards. Rousey has previously spoken publicly about experiencing multiple concussions during her judo and MMA career, a factor that has prompted regulators to require comprehensive neurological review.
The commission is following guidelines consistent with those of the Association of Ringside Physicians for veteran competitors.
What the Medical Evaluation Includes
Before receiving clearance, both fighters must undergo:
Brain imaging scans, including MRI and MRA
Neurocognitive and concussion battery assessments
Comprehensive blood work
Cardiac screening, including EKG testing
Ophthalmologic examinations
CSAC Executive Director Andy Foster has indicated that the bout will only be sanctioned if both athletes pass all required medical protocols without concern.
A Legacy Matchup
The matchup carries historic weight. Rousey was the first woman signed by the UFC and became its inaugural women’s bantamweight champion, helping drive mainstream acceptance of female MMA. Her armbar victories and unbeaten streak early in her career established her as one of the sport’s most recognisable figures.
Carano, meanwhile, was one of the earliest faces of women’s MMA, competing in major promotions before the UFC introduced female divisions. Her influence helped lay the groundwork for the growth that fighters like Rousey would later capitalise on.
Though neither has competed in the cage for years, the bout has generated considerable attention due to its legacy implications and crossover appeal.

Beyond the spectacle, this fight highlights a broader shift in combat sports toward stricter oversight of long-term brain health. Athletic commissions are increasingly cautious with aging athletes and those with documented concussion histories, especially in high-profile events.
For Rousey and Carano, the road back to the cage does not begin with training camp intensity or tactical adjustments. It begins in the medical examination room. Only after satisfying rigorous neurological standards will one of the most talked-about comeback fights in MMA history officially move forward.
