Velocity obsession, early specialization and pressure to perform are driving a new wave of Tommy John surgeries among teenagers

The UCL Crisis: A Century-Old MLB Injury Is Now Hitting Kids the Hardest

When 19-year-old college pitcher Kade Durnin felt a sharp pop in his elbow, he instantly knew something was wrong. The verdict: a torn ulnar collateral ligament (UCL) — the injury that has haunted pitchers for nearly 100 years.

Once associated with elite MLB players, UCL tears — and the Tommy John surgeries used to repair them — are now surging among teenagers. Yankees head physician Dr. Christopher Ahmad says 8–10 of the 10–15 surgeries he performs each week are on high-schoolers, a dramatic shift from two decades ago.


The Velocity Race

Why the explosion? Young pitchers are simply throwing harder than ever.

The chase for triple-digit radar gun readings has triggered a boom in “velocity enhancement programs,” high-risk drills like pulldowns, and year-round training that pushes immature arms past breaking point.
“I was trying to hit 101 mph,” Durnin admits. “I didn’t hit it — and I got hurt.”

The pressure extends far beyond ambition — it’s tied to hopes of scholarships, draft money, and the dream of becoming the next Shohei Ohtani or Juan Soto.


Overuse, Mismanagement & Burnout

Many teens play multiple positions, pitch too often, and rarely rest. One study found 90% of youth teams violate Pitch Smart guidelines, and young players often feel pressured by scouts and coaches chasing wins or commitments.

Some even reinjure themselves while rehabbing to protect a scholarship offer that later disappears.


The Mental Toll

Despite the surgery’s success stories, UCL tears come with heavy psychological fallout. Research by Ahmad and colleagues shows 30% of injured young players show probable PTSD.
Players describe the year-long rehab as “mental warfare,” fearing they’ll never catch up again.


Baseball’s Dilemma

As MLB grapples with rising injury rates, experts debate solutions: pitch limits, six-man rotations, stricter youth rules. But the sport’s culture still glorifies velocity — the very thing that increases injury risk.

“That’s baseball’s paradox,” Ahmad says.
“What makes you better is exactly what breaks you.”
